Six Steps + Programme
What is the Six Steps + Programme?
St Luke’s Plymouth has adapted the original Six Steps training programme, which started in Cumbria, to the Six Steps + Programme, which is now a continuous quality improvement programme for care homes , nursing homes and domiciliary agencies.
We recognise that most people do not die in the hospice and that the majority of end of life care is delivered in other settings. As a hospice we are committed to supporting others to care for people at end of life, through education and training and helping organisations have the right policies in place to deliver best care.
The work we do within the Six Steps + programme with care homes and other organisations, supports our Hospice without Walls vision and we view our Six Steps + organisations as valuable partners in the delivery of end of life care.
To date 68 organisations and 118 individuals are registered on the programme

Following a EOL Link forum, EOL Champions stated they would like a practical resource web page where they could go to download resources. In response to this request the following pages have been created. The resources are indexed in the format of the Six Steps + eg you will find ACP leaflets in Step 1 etc. We hope you find the pages useful and would welcome your feedback, please email firstname.lastname@example.org with any comments.
