St Luke’s at Derriford
The St Luke’s hospital team provides a specialist care service to Derriford Hospital, seven days a week, across every ward in the hospital. The core team is made up of specialist doctors, nurses and administration support. They work closely with an extended team including a chaplain, physiotherapist, occupational therapist and welfare rights officer.
What do we do?
The team offer specialist advice and support for any hospital inpatient with a progressive life limiting illness. Working alongside the hospital doctors and nurses, they offer advice on complex symptoms and provide emotional support for patients, their family and carers. The team are also heavily involved in delivering education in end of life care to nursing and medical staff across University Hospitals Plymouth NHS Trust.
The team will work as key workers to an individual, ensuring a patient’s care is coordinated and that they have access to all the relevant information and advice and answering any questions, during or after treatment. Complex discharges are arranged through the hospital’s discharge teams, but the St Luke’s at Derriford team will often offer support with this process.
We are based just outside Brent Ward on Level 8.
