Here you can find St Luke’s patient referral forms for our community team, inpatient service and our Urgent Care Service. Furthermore, listed are our referral criteria for your information. If you have any questions regarding referral for a patient, call us on 01752 964200 (7 days a week, 8.30am to 8.30pm). This number will take you to clinical admin who will advise you on the best course of care available for your patient.

Healthcare professional referral criteria to St Luke’s End of Life Urgent Care Service. This service will care for patients at home including those in residential and nursing homes. Service formally known at St Luke’s Crisis Team.
